EMS Star of Life Awards and EMSC Right Care When It Counts Awards

Dear Maryland EMS & EMS for Children Advocates – Trauma Teams

The application process for our EMS Star of Life Awards (Adults) and EMSC Right Care When It Counts Awards (Children & Youth) has opened for 2013. Incidents and activities that occurred between January 1 2012 and December 31 2012 are eligible for submission. This year the forms can be SUBMITTED in three ways.

1- Open the PDF document and complete the application – print and then fax to the MIEMSS Educational Support Office
2- Open the PDF document while on a laptop or computer that is connected to the internet, complete the application and then click on the SUBMIT button.
3- Open the PDF document and complete the application – save the document under a unique name and then email toawards@miemss.org

Deadline is April 1st, 2013.
